I am aware that my child is taking part in this football training program/activity. I am aware that a sport like soccer carries an inherent risk of harm and that occasionally accidents and falls may result in significant injuries to the participant. I indemnify Southern Sydney Soccer from any liability for any damages brought against them in connection of any harm to my child. I attest that my child is healthy and capable of taking part in all activities. I consent to sharing any existing psychological or medical issues with the coaching staff. I authorise a staff member to provide care if it's necessary to treat an illness or injury. I also allow Southern Sydney Soccer's permission to capture pictures or films of the participants for marketing purposes, with the understanding that Southern Sydney Soccer will only use the photos and videos in a professional and ethical manner for promotional purposes. * Required
